In our mission to promote independence and wellness for frail elderly and disabled adults, United HomeCare® embraces interdisciplinary partnerships with physicians and other medical professionals, hospitals and other healthcare facilities and academic institutions.

From care transitions across settings to disease management, telemedicine and other emerging technologies, we are open to collaboration in a number of areas, with the goal of improving the lives of those we serve.
